Seriously, I don't know that lisalu's entry can be topped, but I have a candidate for the Longest Running Ad. I've seen this one over & over for at least 6 months, & someone else said it's been showing up for over a year.

Filthy Machine, $95
This might not be the worst-looking machine I've ever seen (no rust, not obviously missing parts), but the seller deserves some kind of prize for sheer determination. The digital date on one of the photos appears to be 12/28/2009. This same exact ad, with the same 2 photos, and the same abbreviated ad copy, has been posted once or twice a week since about that date -- for the last year and a half.
ETA: Oh, JillyBe beat me to it!! The San Francisco Bay Area CraigList "sewing" listings just wouldn't be the same if I didn't see this ad every week.

The following CL ad was up for less than 12-hours, but I posted it verbatim in another Sewing Machines forum thread a couple months ago:
Singer Treadle Sewing Machine - $7000 (~~city name~~)
This is a beautiful machine. It has the fold out sewing machine.
Also the table has a long slim fold out drawer at the top
that still has 1950 sewing supplies as do all 4 drawers.
It has a cast Iron foot Treadle
I cannot ask any less as this has been in the family for decades.
The pictures do not do this justice.

My comments: The photos didn't do it justice because they were taken from across the room, but it appeared to be a clean, but otherwise unremarkable treadle in an unadorned 5-drawer cabinet. It looked something like this, but the photos weren't as clear. For $7000, it should have been gold-plated.
At the time, I read the ad to my husband. Me: "Hey, listen to this!" (He does NOT have SMAD, but seems understanding of my own SMAD condition.) He was drinking a cup of tea and nearly did a spit take, then burst into laughter when he heard the price.
