Each review only has a single thumbnail. If you want to show different angles of the same item (back, lining, etc), you should include those as links inside the review.
If you have multiple items made from the same pattern and want to show them off, you can also just include links to them in the same review. I you want all of them to show in thumbnails however, you have 2 options:
1) Write multiple reviews, one for each item you made with a separate thumbnail of each item for each review.
2) Use a single review, but make a composite photo in Paint (paint composite tutorial here), or any other photo editing program. I highly recommend experimenting with making composite photos. IT is useful not just for contests but for all reviews, and is a great way to show off many different versions of the same pattern with one easy click for the viewer/voter.
