- Perhaps it is Out of Print
- They have the wrong size
- They have multiples they would like to trade for another

Once that review is done, you can add it to your purchasing wish list or you can put it in your catalog. You can also remove a pattern once you've sewn it. But you have to be a Friend in order to utilize it; it's one of the perks. Now the above might be different since I last used the catalog; there have been some changes and I don't use my catalog much.
I think there is a topic on the board here for a pattern exchange, if I'm not mistaken, but the posts are very old. This idea has been brought up several times.
The Flickr idea is a good one, but Flickr is limited to 200 free photos and I'm sure it would fill up quickly, knowing pattern stashes (like mine). And if PR is going to foot the bill for the additional, our membership rates will have to go up.
