How do you modify your regular patterns?

To modify existing simple patterns like T's, do you widen just the front (A-line shape) and not the back? Adding length, of course, and what about the bustline?

For the popular ruched Maternity styles, how much length do you add for the ruching? I picked up M6751 for one to try for hot summer wear for DIL, who lives far away to fit.

Guess I will go into Motherhood Maternity and measure some widths!
