I don't know if I can explain how I'd draft this but I'll try. I would start with a very short set in sleeve. It would be set in to the armhole from about the balance marks downward. I would make sure the sleeve cap had no ease in it. I would use this as the undersleeve. I would trace it off a second time and slash through the pattern vertically at intervals, spreading the pieces so that there was a small amount to gather at the top but a larger amount at the bottom edge. In other words, the pieces would be spread apart more on the bottom edge. I'd make the second pattern a bit longer. I'd stitch these two so that the inner lining which is shorter holds the puffiness of the outer sleeve. I'd have to experiment with how much puff to put in, and would probably use a light weight fabric for the outer and a firmer one for the inner. If the sleeve wouldn't sit right over the shoulder, I'd make a very narrow band where the edge of the shoulder of the bodice back and front would normally be, and set it into this. Often though, these stay up ok.
