how do I fix the hole over a knee? my childrens pants are small and its so awkward and near impossible to fix it.

also, does thos the patch go over or under? what about when the fabric isn't jeans? what about when its knit or fleece? then how do I fix it? How do I stop it from fraying and tearing even more?

Ah, yes! A wonderful memory when I had to patch DS's pants. You need to rip out the inside seam so you can get your machine in there to stitch. There are some iron on patches you can use, but they don't hold up well in the washer and will come off eventually. For the jeans, I always put the patch on the outside, mostly because the jeans fabric is pretty durable and you want something as strong in the hole-y area. Knit or fleece will require more knit or fleece, and I suggest you stitch that from the inside. To keep it from fraying, try fusing some interfacing to the piece you are cutting out as a replacement. Then zigzag around the edges to help prevent fraying even more. Then sew on the patch from the inside.

Yep, still have a baggie of those iron on patches- old fashioned large ones. I usually pulled it from the dryer, noticed knee tear, added it to my ironing pile, ironed on the patch (yes- the edges always pull away) then tossed it in mending pile where I sewed around the entire iron on. But the fabric sometimes ripped above or below it.
I also saved old jeans to make patches for the knees- works best if you put on jeans or corduroy or leather like patches BEFORE even worn. The better give that the patch material has, the less chance of ripping above or below the patch.

I bet if you iron on some soft stretch interfacing to the inside of the fleece knee area and stitch around it when you buy it- the knee tears will be reduced.
