Because it must be very difficult to get RTW that fits well, you want, even need, to alter patterns, right? My left shoulder is about a 1/2 inch higher and narrower, but the right almost matches the big 4, so comparatively easy adjusments. I think I also have forward shoulders, but don't usually like the results when I adjust for that, so maybe not.
Your pattern adjustments will require you to cut the four bodice pieces separately, in one layer of fabric. For your right shoulder, assuming 1/4 inch for forward shoulder adjustment, you will need to raise the end of the shoulder seam of the right bodice back 5/8 inch, and just 1/8 inch for the right bodice front. Lower the left back shoulder seam in the same way. With a 1/4 inch forward shoulder adjustment, you would lower the end of the shoulder seam of the left bodice back 3/8 inch and the left bodice front 7/8 inch. Augh, fractions.
Garments with padded shoulders, a thicker one on the left, maybe none on the right, could even out some of the difference. You might want to put a left shoulder pad in most of your garments, since that shoulder is 5/8 inch lower than big 4 patterns.
