Butterick 5711 Misses'/Misses' Petite Top and Belt
Loose-fitting top has collar, mock front button band, front/back tucks, gathered sleeve cap, shaped hemline and narrow hem. A: sleeves with underarm slit. B: sleeve bands with button trim. A and C: self belt. D: bias collar extends into tie ends. C and D: sleeves gathered into button cuffs with continuous lap.

Another great blouse pattern. I used woven chiffon, and it came out sooo comfy. It has tucks in the front and the back, and gathered sleeves with a band at the bottom of each. It also has a collar.
