I am making a knit dress for my sister. Her shoulder is only 3". How do I adjust the pattern for this alteration?

You mean the length of the seam from the neckline to the sleeve cap? You can change that length by slashing a line from the shoulder notch to a point midway between the shoulder and neck, then fold out the length, tapering to zero at the seamline of the armscye. Then, you have to add that height to the sleeve cap, and smooth all the lines that you've disrupted.

To get the length to fold out, I'd think you'd need to make a muslin to find out, or at least a tissue fit. I don't think I'd be able to get this particular measurement by flat measuring the pattern pieces.
