My adult son broke his ankle most wonderfully in a bicycling accident, and I have found that The Green Pepper Polar Sock can be adapted to make a good cover or sock for a lower leg cast:
Green Pepper Polar Sock
As a sock, this pattern is kind of unshaped and floppy (Kwik Sew makes one that's more fitted), but if you enlarge it to a sixteen-inch girth and a 20 inch length for an adult cast, and make it of stretchy Malden's Bi-Polar fabric, the vague shaping results in a pretty nice cover to keep the toes warm, and the cast clean.
