I'm trying to fit a pair of muslin pants for a male friend. I'm not sure where to alter but we seems to be getting horizontal creases at the front as well as a "tent" at the crotch...

What is the correct way to alter the pattern?

P.S. He has a full frontal and inner thigh and a slight pot belly...

Horizontal creases mean the pants are too tight--add width to the pattern where he needs it. I'm not sure what you mean about the crotch "tent". Are you fitting slacks or jeans?
Most mens pants (jeans are the exception) are fairly loose around the crotch--keep that in mind when trying to fit him.

We are making flat front trousers.

I think the pot belly is pushing the center front down and making that tent-like excess fabric at the zipper area.

Should I lower the centre front??

You could do that - many men who have a bay window wear their pants below their actual waistband. Or you could slash the front pattern peice from the waist down to the knees and spread it to add more room for his stomach - perhaps an inch at first? Measure him across the stomach from where the side seams would be and compare it to the pattern measurement.
