Has anyone had this issue? How did you fix?

Yes. Which model do you have?
Be sure to use the correct needle (which I believe is ELx705 for all Pfaff coverstitch sergers) and be sure it is inserted all the way up. Sometimes we think it is - and it isn't!
The solution for me was very easy. I had a 4872 and now a 4874, and I just had to be sure to go through ALL THREE needle thread guides before threading the needle -- the instructions didn't specify to do so, but that's all it took!
